    • Professor Robin Miller, University of Birmingham

      Professor Robin Miller is an applied academic with an international reputation in relation to integration and leadership in health and social care. He is the Co-Editor in Chief of the international journal of integrated care and has researched widely the implementation of transformational programmes in primary health and social care. Robin has been P-I or Co-I to over £14 million of research grants over the past 10 years and have published over 60 peer reviewed articles and books. He is the social care lead within the WM Applied Research Collaboration, leads the community asset theme of the national priority programme for adult social care and social work and is a Senior Fellow of the NIHR School for Social Care Research.

      Lecture Title: Social Care: the missing piece of the primary care jigsaw?

      The health of individuals and communities is based not only on the level and quality of primary care services but also wider determinants such as nutritious eating, financial stability, physical activity, safe housing, and access to education and employment. Social care’s role is to work with individuals and communities who face exclusion and inequalities due to factors such as long-term disability, frailty or complex social circumstances. Joint working between primary care and social care professionals and services is therefore key if we are to improve the overall wellbeing of our populations. Historically though, such collaboration has been limited which has negatively affected the quality of care and overall outcomes. In this presentation, Professor Miller will share insights into the challenges of such collaboration and how this can be improved in the future.

    • Mieke Rijken, PhD, Netherlands institute for health services research and University of Eastern Finland

      Mieke Rijken, PhD, conducts research at Nivel (Netherlands institute for health services research), and is a professor of international health and social care at the University of Eastern Finland. She studies the implementation and outcomes of person-centred integrated care for high-need patient populations in primary care and hospital settings, with a specific focus on the identification of high-need patients and patient-reported care experiences and outcomes. She has contributed too many EU projects and actions to improve chronic disease prevention and management (e.g. ICARE4EU, JA-CHRODIS, Selfie2020, EU NCD initiative ‘Healthier together’). She is currently leading the PaRIS-SUR project, which supports the OECD and 20 countries in developing and implementing the international Patient-Reported Indicators Survey (PaRIS).

      Lecture Title: Person-centred integrated care for people with multimorbidity: needs, care models and outcomes.

      Primary care professionals are seeing more and more (older) people with multimorbidity, due to increased life expectancy and the fact that today most older people live independently for as long as possible. In some countries, transfer of hospital care to primary care also plays a role. 
      Awareness has raised that (single) chronic disease management programmes, e.g. to manage cardiovascular disease or COPD, fall short to meet the comprehensive needs of these people and may cause fragmented care. Therefore, in many countries person-centred integrated care models are being piloted and implemented in primary care. This is complicated, because it requires a different way of working, both with patients and with other health and social care providers. And this in turn requires other roles and professional skills, and other organisational and financing models.
      In this presentation prof. Mieke Rijken will discuss the identification of high-need multimorbid patient populations for person-centred integrated care, and provide examples of primary care initiatives targeting such populations in European countries. She will also discuss the current evidence underlying such approaches.

      Director of Quality Improvement and Doctors Health Program at Irish College of General Practitioners, President of EQuiP WONCA Europe.

      Lecture Title: Patients, Professionals, Primary Care and Planetary Health.

      Dr Rochfort will introduce some concepts and terminology used in Planetary Health. She will explore the activities of family medicine and primary care with a planetary health lens. Practical examples of actions that can be taken in primary care by professionals and by patients will be shared, and the co-benefits of caring for our planet while we care for our patients will be described.
    • Marit Vassbotten Olsen, RNA, MNSc, Assistant professor, Western Norway University of Applied Sciences – HVL

      She has worked both locally in the clinical setting and nationally in a special education program for nurse anesthetists, and internationally as a board member of the International Federation of Nurse Anesthetists (IFNA) and its Anesthesia Program Approval Process (APAP) program.

      Lecture Title: Simulation-based learning in education.

      Simulation is used as a pedagogical method in health professional education. The lesson will present various examples of its use - patient observation, teamwork with patients or preparation through virtual reality programs. At the end, we will reflect on why simulation is a useful and important learning method and what it means for clinical practice, patient safety, and most importantly, for the patient.
